How to Complete Your ApplicationThe following information is required on your online application. You will not be able to submit the application until all required sections are complete. You can save your work and come back to complete it later if you wish. Once an application has been submitted you may not change anything.
Tribal Affiliation —submit your CIB or other proof of enrollment. If you are not enrolled but have an enrolled parent or grandparent please submit proof of enrollment for relative and include birth certificates that prove that you are a descendant.
1. Digital photo - Photos are shared with our donors to encourage them to make future donations. Upload a digital photo that’s at least 1.5 MB in size. Scanned photos should be at least 200 dpi resolution. Please be sure it’s a photo that a donor would like to see. 2. Transcripts - Upload your most recent college transcript. Unofficial transcripts are accepted. If you are currently enrolled for the spring semester you may submit your most recent transcript and e-mail or fax us your spring grades when available. High school students should upload your high school transcript into the college transcript field. 3. Financial needs analysis (FNA) form—Available to download from the online application or click here to download the form. Must be completed by your school’s financial aid office. You may upload this form or have your school fax it directly to us.
